COMPENSATION/RULES AND REGULATIONS
All injuries sustained by employees while on duty shall be reported
immediately to the building administrator or to the employee's
supervisor.
Such reports shall include the date, time, and place
of the accident; a description of the injury; the part of the body
affected; how the accident happened; and the names of witnesses who may
be able to verify the circumstances.
Building administrators or supervisors shall send
accident reports to the business office immediately.
